Rent is based on income. The Home accepts Medicaid and will coordinate income options to assist with payment.

Pre-Admission Requirements:

• Chest x-ray
• History and physical
• Signed physician orders if applicable
• Payment arrangements:
  sliding fee scale based upon income
• Safety Background check
